Recruit sites for program implementation and build professional database for future reference. Coordinate and facilitate additional support services as required for program at designated sites. Provide support groups for youth and parents at designated sites. Perform and document intake and assessments on all clients. Provide and completes all DPH programmatic reports. Maintain accurate and timely records. Prepare status reports as required. Provide referrals and follow-up services for additional resources and services as needed. Work closely with Board of Education, teachers, Parent Center, parent aids, and other agencies serving the target population. All other duties as assigned.
