About Boghuma

hello, I am Boghuma I have worked with seniors before and I am currently working with a Health care company and some of the things I do there are developing and/or implementing a comprehensive care plan based assessment and evaluation patient/caregiver needs, I am responsible and accountable for the provision and facilitation of comprehensive care coordination services and quality outcomes for patients, I promote effective utilization and monitoring of health services, I collaborate and communicates with the healthcare team and patient/caregiver to manage care and transitions and when short on staff I go to the homes and there I make sure they take their meds, eat, get some exercise and I come up with some fun activities.
