Community Health Worker Currently, I work full time as a Community Health Worker. In this role, I connect patients, the majority of whom are seniors, with social services, such as food, housing, transportation, socialization, and aging services. I visit their homes to take the patients' vitals and facilitate telehealth visits with Registered Nurses. I am working on my PA Community Health Worker certification. My job and compassion for seniors and members in need of support has given me a good understanding of seniors' needs.
