6/2/2026 Dependable, kept herself away from situations where she may catch a cold. Reported and was on time to all her scheduled shifts and acted responsibly to be available to work other shifts when other workers called out unable to work their shifts on short notice. Implemented a patient logbook, a standard that unfortunately overlooked for months before Patricia came to our employ. Purpose of logbook was to record observations to share with others of patient's physical and emotional condition, equipment function, care services provided, and left questions to hold other caregivers for other caregivers to responsibly respond. Recorded vital readings: blood pressure, pulse, O2, temperature, ounces of patient's actual vital food and hydration intake, and not skewing number by throwing food or nutritious drink away. Focused on bowel movements, urine amount and quality, hydration, feeding nutritious food, sleep patterns, frugal with patient's supplies, not one to waste client's food. Aware of potential of overmedication, has a preference of using simple substances like prune juice and Tylenol as needed, over prescription drugs. Monitored and recorded conditions and function of devices such as Purewick, pressure reducing mattress, adjustable bed, to name a few. Her experience saved our mother's life twice by recognizing the patient was in need of hospitalization and emergency services. Suggested Healthcare Power of Attorney that their fear of death due to transporting via ambulance and hospitalization were worth taking to give patient best shot at surviving. Has sense to identify a patient's symptoms for UTI's. Developed rapport and trust with patient that allowed her to reduce anxiety and effectively feed and clean them. Always made herself available to talk to family about care during off hours. ~Daniel
